Initial Consultation

Office hours are Monday through Thursday, 8:30 am to 5:00 pm. , and Friday from 8:30a.m. to noon. Call (408) 292-1654 x 5 x 5to schedule an appointment.
What you can expect at your first appointment:
After you have been greeted and turned in your new patient forms you will be brought back into the treatment area by one of our friendly staff. After a review of your Health History and any concerns that you may have, we will begin to gather the records needed to establish you as a patient in our practice. This includes taking blood pressure, x-rays, intra-oral photographs and sometimes extra-oral x-rays.
The doctor will then find out from you what you are looking for in a dentist. He will thoroughly explain to you about the teeth and gums so that you can fully understand the exam. When the Dr sits you back he will begin the exam by looking at the soft tissue. This is known as an oral cancer screening. He will then do a pocket charting and see where you are periodontally. (We do not schedule your cleaning appointment until your periodontal condition has been established).
Next he will look at the teeth, listing what he sees and prioritizing any dental work that may need to be done. Finally he will assess your TMJ and your bite. You should find the first appointment is very informative. At the end of your appointment we can schedule you to come back for a dental cleaning. Should you have any questions please feel free to call the office. To save time, you may download the new patient questionnaire and health history forms to fill out ahead of time and bring to the appointment. Be sure to bring any dental insurance information with you at that time as well. We look forward to meeting you.
Your initial appointment will consist of a thorough examination, xrays, photographs and consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- New patient form and packet filled out, you can fill the online registration form to expedite this process
- any available pertinent x-rays
- A list of medications you are presently taking
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the examination visit, unless authorized.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
